2025-2026 Forms
All registrations, payment, and health forms must be completed before September 30th in order to be eligible to participate in practices.
- A complete physical examination signed by a doctor and dated after 4/30/25 is required to be uploaded. Please click HERE
- You must also upload copies of Insurance Cards or apply for school insurance. (Please see Coach Reul in Room 3107 f you need to apply for insurance).
Final Forms and Required Medical Forms
All athletes must complete all required forms in Final Forms by September 30.
You will not be allowed in the water until your file is marked complete. This may take up to 14 days to verify. DO NOT PROCRASTINATE OR YOU MAY BE INELIGIBLE TO SWIM/DIVE.
